CSU Benefits Open Enrollment​ September 11 to October ​6, 2017

The CSU annual Open Enrollment period takes place September 11 through October 6, 2017. Open Enrollment is your annual opportunity to review and update your current benefits for the upcoming year. You can enroll, change or cancel CalPERS health, dental, vision, Flexcash, Dependent Care Reimbursement (DCRA), Health Care Reimbursement (HCRA) or voluntary benefits plans.

Benefits do make a difference! The health and well-being of our employees is important, and we encourage you to make sure your benefits work for you.

The effective date for all changes made during the Open Enrollment period will be January 1, 2018.

To Request an Open Enrollment Change

Complete your Enrollment Worksheet (PDF) and return it to Benefits Services, located in the Human Resources Department at Cal Poly Pomona, CLA Building 98, Room B1-20.  If adding any eligible dependents not previously covered, you will need to provide each dependent's Social Security Number, a copy of marriage certificate if enrolling a spouse, a copy of Domestic Partnership certification if enrolling a Domestic Partner, and birth certificates for dependent children.

Time:  11:00 A.M. - 1:00 P.M.
Location:  CLA Bldg. 98
Area:  Paseo

At the Fair, attendees will be able to meet directly with all of our benefits vendors (medical, dental, vision, life, long-term disability, voluntary insurance programs, and the Tax Shelter Annuity companies, etc.). In addition, there will be exciting exhibits, health screenings (blood pressure, blood sugar, cholesterol and body mass index), mini massages, interactive demos, goodie bags, and raffle prizes for attendees to enjoy.

This is a great opportunity to ask the experts and learn about the many benefits programs.
