Dependent Eligibility Verification

Dependent Eligibility Verification (DEV) is the process of verifying the eligibility of your spouse, domestic partner, children, stepchildren, and domestic partner children (family members) enrolled for state health and dental benefits enrollment. Government Code section 22843.1 mandates that you verify the eligibility of your family members. Government Code section 22959 authorizes the review of your family members' dental benefits enrollment. Verification Schedule

Your family members' eligibility will be verified once every three years. The verification cycle is based on your birth month. The following chart illustrates the schedule

This year CALPERS is auditing  employees born in the month of January, April, July and October, and this is the reason CALPERS is requested this information for your dependents. The Dependent Verification Process is done by our campus.  We first need to approve your documents and then verify your dependents.  You can submit all of your documentation for the Dependent Verification online with two different systems.  Please see the instructions below.

How to Verify Dependents?

Please submit your Dependent Verification Affidavit by logging into DocuSign with your CPP credentials.  Complete the form and submit.

The second step is to submit your supporting documents (marriage certificate, proof of marriage, certified birth certificate) by logging into PolyDoc and uploading the documents there.

What is considered a supporting document?

CalPERS is requesting to show “proof of marriage,” meaning that with your marriage certificate, we will also need something showing that you still live in the same household and are still married. Examples of proof of marriage are last years tax returns, a bill showing both spouses names and their shared address (within the last 60 days), a check showing your names and address or some type of linked account with both of your names and  shared address. Any of these documents will suffice the requirement.

What Documents are Required?

Employees verifying their spouse need to provide a certified marriage certificate and a supporting document

MARRIAGE CERTIFICATE              

A copy of the government issued marriage certificate or domestic partnership registration filed with the California Secretary of State or a comparable agency in another jurisdiction listing the family member as the domestic partner.

EXAMPLE OF A SUPPORTING DOCUMENT:

A copy of the first page of the subscriber's federal or state income tax return from the previous tax year listing the subscriber and the spouse or domestic partner.

OR

A combination of other documentation, including but not limited to, a household bill, account statement, or insurance policy listing the name and address of the subscriber and the spouse or domestic partner, or other documents that substantiate the existence of a current marriage or domestic partnership. Household bills and account statements older than 60 calendar days are unacceptable.

*The first document establishes the life event allowing the enrollment of the dependent (i.e., marriage or registering as domestic partners). The second document substantiates the relationship is current.

Employees verifying their domestic partner need to provide a declaration of domestic partnership and a supporting document

DECLARATION OF DOMESTIC PARTNERSHIP          

A copy of the government issued marriage certificate or domestic partnership registration filed with the California Secretary of State or a comparable agency in another jurisdiction listing the family member as the domestic partner.

EXAMPLE OF A SUPPORTING DOCUMENT:

A copy of the first page of the subscriber's federal or state income tax return from the previous tax year listing the subscriber and the spouse or domestic partner.

OR

A combination of other documentation, including but not limited to, a household bill, account statement, or insurance policy listing the name and address of the subscriber and the spouse or domestic partner, or other documents that substantiate the existence of a current marriage or domestic partnership. A copy of birth certificate naming the subscriber as the parent.
