Complete your E-Signature Authorization

Don't forget!  Complete your e-Signature Authorization form on PolyDoc by logging in with your CPP credentials.  Having the form on file will eliminate the need for employees to return and sign benefit enrollment documents for all transactions including: new enrollment, open enrollment changes, and any changes resulting from a family status change for all benefit transactions.

To Request an Open Enrollment Change

Complete your Enrollment Worksheet on PolyDoc by logging in with CPP credentials.  If adding any eligible dependents not previously covered, you will need to provide each dependent's Social Security Number, upload a copy of marriage certificate if enrolling a spouse, a copy of Domestic Partnership certification if enrolling a Domestic Partner, and birth certificates for dependent children.
