I am requesting assistance from the AVP/DOS Office. By completing any of these forms, I am agreeing to receive additional information via email from this office. As we are mandated reporters, I also understand that information provided may require a referral to, but not limited to: Campus Police, CAPS, the Office of Student Success, the Office of Equity and Compliance, etc.
