Submitting Forms

For the safety of your own information, please do not email documents containing sensitive information to our office.  Please review the upload instructions by aid year (below) to locate and submit specific financial aid forms.

Please note, students and families will be unable to visit our in-person location in the Student Services Building until further notice. Please continue to monitor the CPP Health Alert website for updates to this information.

2020-2021 Forms

Providing the best student experience is important to us. We want you to be able to complete your financial aid file on your own time and on the device of your choice. If you completed a Free Application for Federal Student Aid (FAFSA) or a California DREAM Application, you are able to:

  • Complete financial aid forms on your phone, tablet or computer
  • Securely upload documents from any device
  • E-sign documents - both you and your parents
  • Manage your financial aid tasks online
  • Receive automated text/email reminders about outstanding tasks and next steps

Most financial aid forms are completed electronically and can be found on the Financial Aid Student Forms site. If for some reason you are unable to create a Financial Aid Account or upload documents, please contact our office for assistance at (909) 869-3700, or Email us at finaid@cpp.edu.

To submit forms to the Financial Aid & Scholarships office, follow these steps:

  1. Visit the Financial Aid Student Forms website (https://cpp.verifymyfafsa.com
  2. Create your account (you only need to create your account once).
    Follow the specified instructions to verify your identity as reported on your FAFSA (first name, last name, date of birth, SSN), or your California DREAM Application (first name, last name, date of birth, CSAC ID)
  3. Once logged in to the Financial Aid Student Forms portal, complete all assigned tasks and securely upload required documentation, or select the request image button to select and appeal or other form.

Making a Loan Request (2020 – 2021)

PLUS Loan Request Forms
The Parent and Graduate PLUS Loan Request Forms are now available through Financial Aid Student Forms Portal. For information on how to make a loan request on the Forms Portal, please read these instructions.

Private Loan Request Forms
FAFSA & DREAM Filers: The Private Loan Request Form is available through our Financial Aid Student Forms Portal. For information on how to make a loan request on the Forms Portal, please read these instructions.

Students Who Have Not and Will Not File a FAFSA or DREAM Application: If you have not, and will not, file either the FAFSA or DREAM Application, you will not be able to access the Student Forms Portal. To access the Private Loan Request Form, email us at finaid@cpp.edu, and put “Private Loan Request – No FAFSA” in the Subject Line. We will then email you the Request Form.

Appeals (2020 – 2021)

SAP Appeals

    • To request a SAP Appeal, log on to (or create) your account on the Financial Aid Student Forms site.   If you have not logged into your Financial Aid Account before, you will need to create an account (To create an account, enter your name exactly as it appears on your FAFSA or DREAM Application).
    • Select the  add iconbutton for SAP Appeal.
    • After selecting the appeal enter a short description.
    • Follow the directions to complete the electronic form. 
    • Upload supporting documentation.
      • Additional Documents for Maximum Time Frame Appeal:
        • If you are submitting an appeal related to having reached the Maximum Time Frame, you must also submit a Timetable to Degree Completion, which must list each of your Required Courses for your degree and the term you will be taking the course(s).
      • Additional Documents for Disqualified After Probation Appeal
        • If you are submitting an appeal due to having been disqualified after Financial Aid Probation, you must also submit an Academic Plan that details the courses that you will complete over the next four semesters. List each course you will register for AND complete. All attempted coursework must satisfy program/degree requirements (including GE’s).
          • Your plan must ensure that any challenges you are currently facing are adequately addressed.
          • AT A MINIMUM, you must successfully complete all listed coursework on your Plan with a passing grade and maintain a term GPA of 2.0 for Undergraduate students (3.0 for Post-Baccalaureate students)

Family Contribution Appeals

Income Reduction appeal or Non-Reimbursed Medical/Dental/Vision appeal or Support of Extended Family Members

    • To request this appeal, log on to (or create) your account on the Financial Aid Student Forms site.   If you have not logged into your Financial Aid Account before, you will need to create an account (To create an account, enter your name exactly as it appears on your FAFSA or DREAM Application).
    • Select the add icon button on for Family Contribution Appeal.
    • After selecting the appeal enter a short description.
    • Complete the electronic form and e-sign it. If you or your parents are unable to e-sign the form, download it and sign it. 
    • Upload supporting documentation as outlined on the appeal form you completed.
    • Finish your appeal by clicking the “submit” button at the bottom.
      • Appeals will be reviewed in the order received. You will receive email notification of our decision.

Dependency Status

    • To request this appeal, log on to (or create) your account on the Financial Aid Student Forms site.   If you have not logged into your Financial Aid Account before, you will need to create an account (To create an account, enter your name exactly as it appears on your FAFSA or DREAM Application).
    • Select the add icon button for Dependency Appeal.
    • After selecting the appeal enter a short description
    • Follow the directions to complete the electronic form. 
    • Upload supporting documentation.
    • Finish your appeal by clicking the “submit” button at the bottom.

Cost of Attendance Appeal (Available beginning July 4):

Appeal for adjusting Financial Aid Cost of Attendance for any of the following:

Additional housing or transportation costs; dependent care expenses; disability-related expenses; computer purchase or additional books and supplies beyond what was included in original Cost of Attendance Budget; Special Projects related to your program; etc.

    • To request this appeal, log on to (or create) your account on the Financial Aid Student Forms site.   If you have not logged into your Financial Aid Account before, you will need to create an account (To create an account, enter your name exactly as it appears on your FAFSA or DREAM Application).
    • Select the add iconbutton on for Cost of Attendance Appeal.
    • After selecting the appeal enter a short description
    • Follow the directions to complete the electronic form. 
    • Upload supporting documentation.
    • Finish your appeal by clicking the “submit” button at the bottom.
