Paper Forms

  • Class Withdrawal Request Form (PDF)
    • Instructions for CBA Students
      • Check Important Dates and Deadlines
      • Fill out form (neat, legible handwriting) and attach supporting documentation.
        • What does serious and compelling reasons mean?  A serious and compelling reason means that the student needs to drop classes for reasons outside the student's control.  Examples include: accident, illness, death in the family, military service and legal issues.  
        • Supporting documentation (Ex: police report, doctor's note, death certificate, letter from the military, lawyer, etc.) needs to be attached to the form.
      • If the student is not dropping all of their courses, please state why they are only dropping the courses listed on their form and why they want to keep the other courses on a separate document (typed or neat, legible handwriting).  
      • Get Instructor's signature.
      • Bring form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain signatures for student (Department Chair, College Dean, and the Office of Student Success).
        • The student should continue to attend classes and submit coursework until all three levels have approved the request. 
  • Course Substitution or Acceptance of Transfer Coursework Request (PDF)
    • A student may use a Course Substitution form when they believe that the work they did in a class at a previous institution is comparable to a course at Cal Poly Pomona.  They will need to provide a course syllabus.  They can also use the course substitution form if they want an unrestricted elective course taken at Cal Poly Pomona to count for a major requirement.
    • Instructions for CBA Students
      • Make sure the course from their previous institution is posted on their Transfer Credit Report.
      • Fill out form (typed or neat, legible handwriting) and attach course syllabus.
      • Bring the form and course syllabus to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain the signatures for student (Department Chair of the CPP course that is being required to be substituted, Academic Advisor, Department Chair, College Dean, and in some circumstances the Office of Student Success). 
  • General Academic Petition (PDF)
    • General Academic Petitions are used for other items that are requiring petitioning that cannot otherwise be completed through a course substitution form.
    • Instructions for CBA Students.
      • Fill out form (typed or neat, legible handwriting).
      • Bring the form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain the signatures for student (Academic Advisor, Department Chair, College Dean, and in some circumstances the Office of Student Success). 
  • Grade Forgiveness Form (PDF)
    • A student may seek grade forgiveness through repeating a course.  When they ask for forgiveness for a course, the grade and the units will not be calculated into the GPA.  The forgiven grade will appear on the transcript.  However, after the grade forgiveness is completed the new grade will be used in the GPA calculation.  It is the student's responsibility to seek grade forgiveness; it is not done automatically. 
    • A student is allotted 16 semester units of Grade Forgiveness (or 24 quarter units).
    • Instructions for CBA Students.
      • Repeated the course and received a grade.
      • Fill out form (typed or neat, legible handwriting).
      • Bring the form to the CBA Student Success Center in Building 164, Room 1060 for the advisor's signature and submit the form to the Registrar's Office. 
  • GWT Waiver Form (PDF)
  • Minor Program Course Approval (PDF)
    • A student may use a Minor Program Course Approval form when they need to substitute a required minor course.
    • Instructions for CBA Students.
      • Fill out form (typed or neat, legible handwriting).
      • Bring form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain signatures for student (Department Chair of Student's Minor and College Dean of Student's Minor).
  • Petition to Exceed Unit Cap (PDF)
    • A student may use a Petition to Exceed Unit Cap if they want to go over 18 semester units.
    • Instructions for CBA Students.
      • Fill out form (typed or neat, legible handwriting).
        • On the left column, enrolled courses. 
          • Students at the time must be enrolled in a total of 18 units.
        • On the right column, waitlisted course(s) and what they plan to enroll in.
      • Students must have a GPA of 2.5 or above in Major Core, CPP, and Overall categories upon approval of the petition.
      • Bring the form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain the signatures for student Academic Advisor, Department Chair, and College Dean).
  • Petition to Waive Restrictions on Course Repeats (PDF)
    • A student cannot repeat a course that you passed with a "C" or better.  They are also only allowed to take a course for a maximum of three attempts. 
    • Instructions for CBA Students.
      • Fill out form (typed or neat, legible handwriting).
      • Bring form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain signatures for student (Department Chair, College Dean, and the Office of Student Success).
  • Schedule Conflict Override Petition (PDF)
    • A student may use a Schedule Conflict Override Petition when the meeting times of two classes overlap. 
    • Instructions for CBA Students.
      • Fill out form (typed or neat, legible handwriting).
      • Get Instructor's signature.
      • Bring form to the CBA Student Success Center in Building 164, Room 1060.  The CBA Student Success Center will obtain signatures for student (Department Chair and College Dean).
  • VA Education Plan (PDF)
    • Instructions for CBA Students.
      • Filll out form (typed or neat, legible handwriting).
      • Bring the form to the CBA Student Success Center in Building 164, Room 1060 for the advisor's signature and submit the form to Sam Kim, Veteran Certifying Official, at the Registrar's Office.
