Frequently Asked Questions

The change of major forms for Psychology and Sociolgy are now submitted via E-FORM on the Registrars website under Forms & Services.

  • Fill out a Course Substitution form (located in the Forms tab).
  • Attach a Course Description from the other college catalog that explains the course content.
  • Have the Department Chair of the Cal Poly Pomona course sign the first part of the form.
  • After the Department Chair of the Cal Poly Pomona replacing course has signed the first part of the form, have your Major Department Chair and College Dean sign the form.
  • If the form is approved with all necessary signatures it must then be taken to the Registrar's Office for processing.

  • Fill out a Course Substitution form (located in the Forms tab).
  • Attach a Course Description (found in the CPP course catalog) of the course you took that you would like to count for the new course that explains the course content.
  • Have the Department Chair of the Cal Poly Pomona course sign the first part of the form.
  • After the Department Chair of the Cal Poly Pomona course has signed the first part of the form, have your Major Department Chair and College Dean sign the form.
  • If the form is approved it must then be taken to the Registrar's Office for processing.

  • Fill out a Grade Forgiveness through Course Repetition Form (located in the Forms tab).
  • Take the completed form to your Academic Advisor for their signature.
  • If the catalog course number has NOT changed, the Department Chair DOES NOT need to sign the form and you can take the form directly to the Registrar's Office for processing.
  • If the catalog course number HAS changed (Ex. 1st attempt: BHS 204-2nd attempt: PSY 204), then the Department Chair signature is required. After, the Department Chair's signature is obtained, you can then take the form to the Registrar's Office for processing.
  • NOTE: You cannot complete a Grade Forgiveness through Course Repetition Form if you received a W in the course.

  • Fill out a Request for Class Withdrawal for Serious or Compelling Reasons Form (located in the Forms tab).
  • Obtain the Course Instructor's signature.
  • Turn the form into the Psy/Soc Department office and the office staff will submit your request to the Department Chair for signature.

  • The Leave of Absence is now submitted via online on the Registrars website under Student Forms & Services.

  • Fill out a Request for Award of Minor Form (located in the Forms tab).
  • We recommend that you complete Part II of the form with the minor coordinator. Please meet with the coordinator of the minor that you are appying for.

Beginning Fall 2013 quarter, students may now pursue one additional major and/or a maximum of two minors if all declared programs may be completed within 36 units above the units required for their primary major. Students must submit a plan demonstrating that they can meet this restriction with their change of major form or minor course approval form. The plan must be verified and signed by the department chair and associate dean associated with the major or minor.
