The CSU offers benefits eligible employees the choice of two dental plans: Delta Premier or Delta Care PMI (http://www.deltadentalca.org/csu/). The premium for dental insurance is fully paid by the state.
The Delta Premier dental plan allows employees to choose any dentist, but the benefits are guaranteed only when the employee and/or family member goes to a member dentist (97% of the dentists in California are member dentists). An employees current dentist may participate in the Delta Premier plan; if so, he/she has claim forms and will file the employees claims. Delta Premier will pay the dentist and notify the employee of any remainder they owe the dentist. If the dentist is not a Delta Premier member, the employee pays the dentist and completes a claim form for reimbursement from Delta Premier . Evidence of coverage (EOC) booklets which detail coverage and plan limitations are available from the Customer Service Center located in Human Resource Services, CLA Building 98, Room B1-20.
If an employee chooses the Delta Care PMI, the employee selects a PMI dentist at the time of enrollment and the employee and all of their covered dependents must use that dentist. (Employees may change dentists by contacting Delta Care PMI). Delta Care PMI will send employees an identification card which the employee will show their dentist to receive benefits; no claim forms are required. All covered dental services deemed necessary by the Delta Care PMI dentist will be provided subject to plan limitations explained in the evidence of coverage booklet (Basic or Enhanced plan) which is available from the Customer Service Center located in Human Resource Services, CLA Building 98, Room B1-20.
The employer pays the full premium cost for benefits eligible employees, regardless of the number of dependents enrolled.
