Request to Establish or Adjust a Campus Based Fee
A completed application will include:
- Memo from their respective Vice President endorsing the request.
- Confirmation that the Chancellor has been notified and has approved the fee proposal. The department must coordinate with its respective Vice President and the Office of the President for all fee proposals.

Divisions and/or units interested in submitting applications to the Fee Advisory Committee for review must submit the above-mentioned materials no later than the Wednesday before their intended presentation / committee meeting date. See below for a list of Fee Advisory Committee meeting and application/material due dates.
