To enroll or renew your health insurance plan, please visit the United Healthcare website to purchase the insurance plan, and view the benefits. Please follow the instructions below:

1. Choose Medical - Low Student Only Plan, and click Explore Policy. 
2. If you don't have a U.S. address at the time when you purchase the health insurance plan, please use Cal Poly Pomona's address: 3801 West Temple Avenue, Pomona, CA, Zip Code: 91768 to make the purchase. You may change the address and zip code information when you have a U.S. address later.
3. Create an account and log in, or start as a guest and create an account later.
4. Select Cal Poly English Language Institute under What insurance category best describes you?
5. Select specific dates that you want to be covered by the health insurance plan. Please note that you must have coverage throughout your whole stay in the U.S. (from day one to the last day before your departure).
6. Provide your personal information and proceed with making the payment.

International students are REQUIRED to purchase this health insurance plan for each term that they attend Cal Poly English Language Institute (CPELI).
