I took the same course twice because I did not pass it or get an adequate grade(C-, D+, D, D-, F, WU, or IC) the first time and I would like to replace the grade I originally received. How do I replace the first grade with my now passing grade?

• A Grade Forgiveness form is available on the Registrar's website under Student Forms & Services.
• Take the completed form to your Academic Advisor for their signature.
• If the catalog course number has NOT changed, the Department Chair DOES NOT need to sign the form and you can take the form directly to the Registrar's Office for processing.
• If the catalog course number HAS changed (Ex. 1st attempt: BHS 204-2nd attempt: PSY 204), then the Department Chair signature is required. After, the Department Chair's signature is obtained, you can then take the form to the Registrar's Office for processing.
• NOTE: You cannot complete a Grade Forgiveness through Course Repetition Form if you received a W in the course.

I am currently taking a course and I would like to Withdraw, but the Withdrawal period to receive a W has already passed. At this point, what can I do to still receive a W in the course?
