This form is intended for completion by a HEERA Manager or their designated representative. 

Please submit this form if: 
  • A single employee, multiple employees, or an entire department is to report to the same HEERA Manager.
  • If employees are to report to different HEERA Managers, each reporting arrangement requires a separate form.
